    When reporting a claim, please provide:

    Identification Information
    • Insured name and policy number
    • Insured contact name, phone number, fax number and best time in which to call
    • Broker or Agent’s contact name, phone number and fax number

    Reporting Information
    • Any summons or complaints
    • Any letters seeking or demanding money or services from the Insured
    • Copy of Insured’s contract relating to the project at issue
    • Copy of project correspondence relating to issue triggering notice
    • Plans or specifications relating to the issue triggering notice
    • Photographs or physical evidence relating to the issue triggering notice
