When reporting a claim, please provide:
Identification Information
- Insured name and policy number
- Insured contact name, phone number, fax number and best time in which to call
- Broker or Agent’s contact name, phone number and fax number
Reporting Information
- Any summons or complaints
- Any letters seeking or demanding money or services from the Insured
- Copy of Insured’s contract relating to the project at issue
- Copy of project correspondence relating to issue triggering notice
- Plans or specifications relating to the issue triggering notice
- Photographs or physical evidence relating to the issue triggering notice
